The Veiovis Direct enrollment process is fast, easy, affordable and simple. It will only require a few minutes of your precious time. Simply fill out the Veiovis Direct Subscriber Form completely, select your preferred payment option and enter your payment method, sign and date at the bottom of the page. If the monthly payment option is selected, the Direct Payment Authorization form must be filled out, signed and dated.

After completion, the Subscriber Form can be submitted by the following methods:
- Scan and email securely to customerservice@veiovis.com or directly to tria.paulino@veiovis.com
- Postal Mail to Veiovis P.O Box 6578 Tamuning, Guam 96931
- Or simply drop off your forms and payment to the Veiovis Direct or Patient Services Department at the FHP Health Center or TakeCare Customer Service located on the 1st floor of the Baltej Pavillion.
